Our social workers deliver essential case management services to individuals and families, coordinating and facilitating services to meet fundamental needs such as housing, healthcare, addressing social isolation, and more.

Transportation

The Mountain Town Connector is our  transportation program. The service offers on-demand rides and scheduled group trips at no cost to the rider in a private car or our wheelchair-accessible AWD Ford Transit van. In addition, the unique program integrates access to social work and advocacy services already provided by our organization.

The Meeting Place

Neighborhood Connections delivers and supports various community programs and services through our space, the Meeting Place. This sizeable multi-purpose activity room is utilized to offer programs that align with community interests, including a monthly meal and exercise classes. In addition, local groups host meetings and social activities.
